Confidentiality
Patient data will only be used in the Registry after the patient signs the IRB-approved informed consent document.
To maintain subject confidentiality, all subjects are referenced by a 4-digit Registry Identification Number. To further protect subject's data, the original source documentation for the subjects will not be shared with the various physicians participating in the IDEaL Registry. Browsing through the database will not be permitted. However, physicians will have access to all data submitted on any subjects they enrolled in the registry.
All participating physician information is confidential.
